The first news concerning Castel Sant'Elmo dates back to 1275. In 1329 Roberto d'Angiò entrusted the task of its extension to the Sienese sculptor and architect Tino di Camaino who transformed the building into a real palatium for the king and for the court, with a quadrilateral plan, with two towers; in 1348 it is defined in documents as castrum Sancti Erasmi, due to the presence in that place of a chapel dedicated to Sant'Erasmo.

In 1456 an earthquake caused the collapse of the towers and some curtain walls with related restoration interventions by the Aragonese. During the Spanish viceroyalty (1504-1707) the castle, called Sant'Ermo and then Sant'Elmo, perhaps due to the corruption of the name Erasmo, was transformed into a defensive fortress by Don Pedro de Toledo (viceroy from 1532 to 1553) and the project entrusted to Pedro Luis Escrivà, military engineer of Valencia. The construction of the building in the current configuration, with a stellar plan, began in 1537 and in 1538 the epigraph was placed on the entrance portal, surmounted by the coat of arms of Charles V with the Habsburg double-headed eagle.
